New Clinical Practice Guideline (CPG) Published!

posted on Aug 13, 2025

We are thrilled to announce the publication of CAG's latest Clinical Practice Guideline for the Endoscopic Management of Nonvariceal, Nonpeptic Ulcer Upper Gastrointestinal Bleeding.

Developed with international collaboration, this is the first guideline to specifically address the endoscopic management of bleeding from:

  • Malignant tumors
  • Mallory-Weiss tears (MWTs)
  • Dieulafoy’s lesions
  • Gastric antral vascular ectasia (GAVE)

The guideline offers 19 conditional recommendations to guide evidence-based clinical decision-making, including:

  • Malignant tumors: Topical hemostatic agents over conventional therapy, followed by oncologic treatment.
  • MWTs: Endoscopic band ligation or through-the-scope clip for active bleeding (spurting or oozing); no endoscopic therapy for nonbleeding visible vessels, adherent clots, clean-based ulcers or flat-pigmented spots.
  • Dieulafoy’s lesions: Mechanical modalities or contact thermocoagulation over epinephrine injection alone.
  • GAVE: Endoscopic band ligation over argon plasma coagulation.

Globally Endorsed

This guideline has been formally endorsed by leading global societies, including the American Society for Gastrointestinal Endoscopy, American Gastroenterological Association, European Society of Gastrointestinal Endoscopy, Sociedad Interamericana de Endoscopía Digestiva, and World Endoscopy Organization.

By providing evidence-based recommendations, CAG aims to support clinicians, patients, and healthcare systems in delivering high-quality care and improving patient outcomes.
